Complete the onboarding kit above and submit along with the following additional required documents - each of these documents must be included to bind a policy and set up commission payments:

1. Application for your first piece of business (or Broker of Record Change documentation)
2. A copy of your insurance license for the state you’re submitting your first piece of business in
3. Completed W9 (must be in the same name as the license submitted) A blank W9 is included in the kit above
4. Executed Producer Agreement as included in the kit above
5. Proof of E&O coverage
